About the Disembarkation Card
This form must be completed when entering Japan. Please fill it out in either Japanese or English.

Disembarkation Card About the Residence Card
Students enrolled in long-term programs will receive their Residence Card at the Immigration Services Agency office located in Haneda Airport or Narita Airport.
The Residence Card serves as your official ID during your stay in Japan.The Residence Card includes the following information: name, date of birth, gender, nationality/region, address, residence status, period of stay, type and date of permission, card number, date of issue, expiration date, and whether there are any work restrictions.
At the same time, please be sure to apply for a “Permission to Engage in Activity Other Than That Permitted” at the airport.
If approved, a stamp indicating this permission will be placed on the back of your Residence Card.
You can also apply for this permission at a later date; however, the process may take time, so we strongly recommend applying at the airport.

If you lose your Residence Card or it is stolen, please contact your school and the police immediately. If your Residence Card is lost, stolen, destroyed, severely damaged, or defaced, you must apply for a reissuance at a regional Immigration Services Bureau office.
If your Residence Card is lost, stolen, or destroyed, you must apply for a replacement within 14 days from the date you become aware of the loss (or from the date of re-entry if you became aware of it while overseas).
When applying, instead of presenting your Residence Card, please bring supporting documents such as a Certificate of Loss Report or Theft Report Certificate issued by the police, or a Disaster Certificate issued by the fire department.
What kind of items should I bring when going to Japan?
You will need to bring your passport, mobile phone, cash, and a credit card. A dictionary in your native language can also be helpful. Clothing and food can be purchased near the school.
